If you require a copy, one can be found in the President's mail slot at 4-Rinks.

Police Check Session Saturday, September 29, 2007

The Mississauga Hockey League will host a police check session with a staff member from the Peel Regional Police Record Search Unit on September 29. Please advise any volunteers who have not completed their police check of the following information and remind them that they must bring 2 pieces of government issued identification (one of which must have a photo). Forms will be made available at the session. Once you receive your police check back in the mail, proceed to STEP 4 below.

If you cannot make the Sept 29th session, Complete STEPS 1-5.

STEP 1: Retrieve a Police Check Form.

STEP 2: Complete

STEP 3: Deliver to The Police Station - Address is on the back of the form.

STEP 4: After you return from the police station you will have been given a receipt for submitting your police check. Place this receipt in the small black mail box on the left side of the Coaches Cupboard or in the mail slot of the House League Director. (See photo below.)

STEP 5: When the legal police check is returned to you by mail. Then this needs to be forwarded in either the small black mail box on the left side of the Coaches cupboard or in the mail slot of the Director of House League. (See photo below.)
